About the Reviewer

This is the question I get most often from patients. They've been prescribed tadalafil — great — but then they stare at two bottles with different doses and wonder which regimen their life actually fits. The honest answer: it depends on a handful of factors that are very specific to you. Let me walk through both options clinically, not just superficially.

Factor 5mg Daily 10–20mg On-Demand
Spontaneity ✔ High Plan 30–60 min ahead
Peak plasma level Steady-state (~5 days) 2 hours post-dose
Effective window Continuous (24/7) Up to 36 hours
Avg. monthly cost (generic) $15–$30 $10–$25 (per 4 tabs)
BPH symptom relief ✔ FDA-approved Not indicated
Side effect intensity Generally milder More pronounced at 20mg
Good for infrequent sex Less cost-efficient ✔ Better fit

How the 5mg Daily Dose Works Physiologically

Tadalafil has an elimination half-life of approximately 17.5 hours. Take it at the same time each day and within 5 days you reach a pharmacokinetic steady-state — meaning the concentration in your blood plateaus at a consistent, therapeutically active level. What you're not doing is flooding your system with a surge of drug every time you want an erection. The background level of PDE5 inhibition is low-grade but continuous.

This matters clinically for a few reasons. First, the side effects — back pain, myalgia, headache, nasal congestion — are dose-dependent. At 5mg, most men tolerate daily dosing exceptionally well. Second, that steady-state concentration is actually associated with endothelial improvement over time. Several studies have shown that daily low-dose tadalafil improves endothelial function markers, which is meaningful for men whose ED has a vascular origin (which is most of them).

The On-Demand Model: 10mg vs 20mg

When you take a 20mg tablet an hour before sex, plasma levels peak around two hours post-ingestion. The drug remains above the therapeutic threshold for up to 36 hours — longer than any other oral ED medication on the market. That's the 36-hour "weekend pill" reputation that Eli Lilly built its entire Cialis marketing campaign around.

Clinically, the 10mg dose is often the better starting point for on-demand use. It's the FDA's recommended initial dose for most men. If 10mg is insufficient after two or three attempts, escalating to 20mg is appropriate. Some men find 5mg on-demand adequate — though this is technically off-label since the FDA-approved on-demand doses start at 10mg.

Does food matter?

Unlike sildenafil, tadalafil's absorption is not meaningfully affected by food or alcohol in moderate amounts. You can take a 20mg tablet after a large dinner and expect the same pharmacokinetics as taking it fasted. This is a genuine advantage over Viagra, where a fatty meal can delay onset by an hour or more.

Which Dosing Strategy Do Urologists Actually Recommend?

According to the American Urological Association's ED guidelines, both strategies are equally valid first-line options. The choice comes down to lifestyle. Three questions help clarify it:

There's no wrong answer. Some men switch strategies seasonally or when their lifestyle changes. That's fine — tadalafil's pharmacology accommodates flexibility.

A Note on Starting Treatment

Neither dose works the first time for every man. Tadalafil often requires two to four sexual attempts before optimal response is achieved — partly physiological, partly psychological. Performance anxiety is a real co-factor in ED. If the drug doesn't work the first time, that doesn't mean it's failed. Give it a fair trial under the guidance of your prescribing physician before switching approaches.
